The role of total thyroidectomy in benign thyroid diseases in an endemic area, retrospective analysis of 208 patients

Aim: Total thyroidectomy is one of the surgical procedures that may be chosen in benign thyroid diseases. In our study we aimed to evaluate total thyroidectomy surgery in terms of its complications in Van – Muradiye region where endemic thyroid diseases are reported and to discuss by determining incidentally detected thyroid carcinoma rates. Material and Method: Age, sex, surgery indications, transient-permanent hypoparathyroidism, rates of temporary-permanent vocal cord paralysis were recorded from the files of patients who had total thyroidectomy surgery due to benign thyroid diseases between 2003 and 2010 in Van, Muradiye region. The incidence of thyroid carcinoma was determined from the post-operative pathology reports. Result: 208 patients who underwent total thyroidectomy due to their benign thyroid diseases were included in our study. When it was evaluated with regards to early period complications, post-operative hematoma was developed in one (0.48%) patient. Transient hypoparathyroidism and permanent hypoparathyroidism were detected respectively in 24 (11.53%) patients and in 1 (0.48%) patient. When it was evaluated with regard to vocal cord paralysis, while temporary vocal cord paralysis was detected in one (0.48%) patient, permanent vocal cord paralysis was not detected in any patient. Incidental thyroid carcinoma was reported in 23 (11.0%) patients. Discussion: When we consider that incidental thyroid cancer may be observed more frequently in endemic areas and its higher recurrence rates, we suppose that total thyroidectomy should be the method applied in benign thyroid diseases.

Endemik Bölgede Tiroidin Benign Hastalıklarında Total Tiroidektomi, 208 Hastanın Retrospektif Analizi

Amaç: Benign tiroid hastalıklarında total tiroidektomi en çok tercih edilen cerrahi prosedürdür. Çalışmamızda, endemik olarak benign tiroid hastalıklarının görüldüğü Van ilinin Muradiye ilçesinde yaşayan, total tiroidektomi yapılan hastalarda gelişen komplikasyonlar ve insidental olarak saptanan tiroid karsinom oranlarını belirleyerek tartışmak amaçlanmıştır. Gereç ve Yöntem: Van ilinin Muradiye ilçesinde yaşayan, 2003-2010 arasında benign tiroid hastalıkları nedeniyle total tiroidektomi operasyonu geçiren hastaların dosyaları retrospektif olarak incelendi. Hastaların yaş, cinsiyet, ameliyat endikasyonu, hematom, geçiçi-kalıcı hipopartiroidi, geçici- kalıcı vokal kord paralizisi oranları kaydedildi. Postoperatif patoloji raporlarından insidental tiroid kanser sıklığı belirlendi. Bulgular: Benign tiroid hastalıkları tanısıyla total tiroidektomi yapılan 208 hasta çalışmamıza dahil edildi. Postoperatif, bir(%0.48) hastada hematom, bir (%0.48) hastada geçici vokal kord paralizisi, 24 (%11.53) hastada geçici hipoparatiroidizm, bir(%0.48) hastada ise kalıcı hipoparatiroidizm saptandı. İnsidental tiroid kanseri 23 (%11.0) hastada saptandı. Tartışma: Endemik bölgelerde insidental kanser ve rekürrens oranlarının daha yüksek olması nedeniyle, benign tiroid hastalıklarında total tiroidektominin tercih edilmesi gerektiğini düşünüyoruz.
