The Effects of Adjunctive Doxycycline in Initial Periodontal Therapy to The Gingival Crevicular Fluid Levels of Matrix Metalloproteinase-1

INTRODUCTION: The aim of this study was to compare the effects of adjunctive use of doxycycline in chronic periodontitis patients on the GCF levels of MMP-1 during and after the periodontal treatment.
METHODS: Twenty-four chronic periodontitis patients (12 female and 12 male subjects with ages ranging between 19 to 59) were included in the study and assigned to test(TG) and control groups(CG) of twelve persons each. Patients in the CG received only full mouth disinfection (FMD) treatment whilst TG patients received 200 mg/day doxycycline for the 1st day and continued with a 100 mg daily dose for the rest of the treatment period. Plaque index (PI), gingival index (GI), pocket probing depth (PPD), bleeding on probing (BI), and clinical attachment gain (CAL) measurements were recorded before and after treatment periods. Matrix Metalloproteinase-1 (MMP-1) levels in the gingival crevicular fluid (GCF) were evaluated with Enzyme-Linked Immunosorbent Assay (ELISA).
RESULTS: The clinical parameters reflecting periodontal health were significantly improved in both groups following the treatment procedures (p=0.001). Before the treatment, the MMP-1 levels were lower in the test group compared to the controls. There was a significant increase on the the levels of MMP-1 between pre and posttreatment levels of MMP-1 in test group (p=0.001). There was not statistically significant difference on the levels of MMP-1 between the groups after the treatment (p=0.83).
DISCUSSION AND CONCLUSION: This study demonstrates that adjunctive use of doxycycline effects the MMP-1 levels in chronic periodontitis patients. Further studies are needed to confirm these findings.

Kronik Periodontitisli Hastalarda Başlangıç Periodontal Tedavisine İlave Olarak Doksisiklin Kullanımının Dişeti Oluğu Sıvısında Bulunan Matriks Metalloproteinaz-1 Seviyelerine Etkisi

GİRİŞ ve AMAÇ: Bu çalışmanın amacı, kronik periodontitisli hastalarda başlangıç periodontal tedaviye ilave doksisiklin kullanımının dişeti oluğu sıvısında (DOS) matriks metalloproteinaz-1 (MMP-1) seviyesi üzerine etkisinin incelenmesidir.
YÖNTEM ve GEREÇLER: Çalışmaya 19-59 yaş aralığında, 12 kadın ve 12 erkek olmak üzere toplam 24 kronik periodontitits (KP) hastası dahil edilmiştir. 12 KP hastası test grubu (TG) ve 12 KP hastası kontrol grubu (KG)’nu oluşturmuştur. Test grubundaki hastalara tüm ağız dezenfeksiyonuna (TAD) ilave olarak doksisiklin ilk gün 200 mg devamında ise 100 mg günde tek doz çalışma sonuna kadar reçete edilmiştir. Kontrol grubundaki hastalara sadece TAD uygulanmıştır. Plak indeksi (PI), gingival indeks (GI), sondlanabilir cep derinliği (SCD), sondlamada kanama(SK) ve klinik ataşman seviyesi (KAS) verilerini içeren klinik parametreler tedavi öncesi ve tedavi sonrası kaydedilmiştir. MMP-1 miktarları ELISA yöntemi ile değerlendirilmiştir.
BULGULAR: Her iki grupta da klinik parametrelerde tedavi sonrasında tedavi öncesine göre istatistiksel olarak anlamlı azalma gözlenmiştir (p=0.001).Tedavi öncesinde test grubunda kontrol grubuna oranla anlamlı derecede daha az MMP-1 seviyesi tespit edilmiştir (P=0.001). MMP-1 seviyesi test grubunda tedavi sonrasında tedavi öncesine göre istatistiksel olarak anlamlı artış tespit edilmiştir(p=0.001) Tedavi sonrası test grubu ile kontrol grubu arasında MMP-1 seviyeleri açısından istatistiksel olarak anlamlı fark bulunamamıştır. (p=0.83)
TARTIŞMA ve SONUÇ: Bu çalışmanın sınırları dahilinde kronik periodontitis hastalarında ilave doksisiklin kullanımının MMP-1 seviyeleri üzerinde etkisinin olduğu belirlenmiştir. Bu konu hakkında daha fazla sayıda çalışmaya ihtiyaç duyulmaktadır.
