Effects Of Postoperative Local And Topical Bupivacaine On Pain After Laparoscopic Cholecystectomy (Prospective Clinical Trial)

Aim: Pain control is an integrated part of patient care. The goals of pain relief are to decrease postoperative pain and morbidity, without side effects. The aim of this study was to investigate the effects of postoperative bupivacaine infiltration and the response of patients to postoperative pain. Method: 66 cases with laparoscopic cholecystectomy operations were prospectively investigated. The mean patients’ age was ranging between 24 to 77 years ( mean: 47 years). In 34 patients in the local anesthesia and metamisol group (LAM), were pulverised 20 mL of %0.05 Bupivacaine with both into gallbladder-subdiaphragmatic areas and, also were injected to trocar entry sites at the end of operative procedure. Remaining 32 patient in the metamisol group ( M ). Were given nothing as locally applied anesthetic both groups received metamisol p.6.q postoperatively. The Visual Analog Scale and the Simple Descriptive Scale were explained and applied to the patients by the same physician. The data was collected at 1st, 6th and 18th hours postoperatively. Results: The postoperative pain complaints were found to be fewer in the LAM group in respect to the M group. It was also found that; neither the age, sex, duration of the symptoms nor the peroperative - postoperative complications affected the postoperative pain. It is concluded that, postoperative local and regional anesthesia with bupivacaine after Conclusion: Laparoscopic cholecystectomy is an efficient method of postoperative pain relief.

Laparoskopik Kolesistektomili Hastalara Postoperatif Lokal ve Bölgesel Uygulanan Bupivakain’in Etkisi

Amaç: Ağrı kontrolü operasyon sonrası bir hastanın ayrılmaz parçasıdır. Postoperatif ağrı tedavisinin amaçları, yan etkisiz ağrının giderilmesi ve postoperatif morbiditenin azaltılmasıdır. Bu çalışmanın amacı, postoperatif lokal ve bölgesel bupivacain uygulamasının postoperatif ağrı üzerindeki etkisini ve hasta özelliklerinin bununla ilişkisi araştırıldı. Yöntem: Laparoskopik kolesistektomi ameliyatı uygulanan 66 olgu prospektif olarak çalışmaya alındı. Çalışma kapsamına alınan olguların yaş ortalaması 47.01?12 (24-77) idi. Hastalar iki gruba ayrıldı. Bir gruba lokal anestezi ve metamizol; postoperatif safra kesesi lojuna ve subdiafragmatik alana püskürtme(bölgesel), trokar giriş yerlerine infiltrasyon şeklinde 20 ml %0.05’lik bupivacain verildi. Diğer gruba Metamizol verildi. Postoperatif dönemde her iki gruptaki hastalara 4-6 saat aralıklarla metamizol uygulandı. Lokal anestezi ve metamizol grubu (LAM) 34, metamizol (M) grubu 32 hastadan oluşuyordu. Hastaların postoperatif ağrı şiddetleri vizüel analog skala (VAS) ve basit tanımlayıcı skala (BTS) ile 1.,6.,18.saatlerde anamnezi alan ve hastayı bilgilendiren hekim tarafından dolduruldu. Bulgular: Hastaların postoperatif ağrı şiddetleri LAM gurubunda M grubuna göre daha düşüktü (p<0.01). Hastaların yaş, cinsiyet, risk faktörleri, semptomların süresi, nazogastrik tutulma süresi, geçirilmiş operasyon, peroperatuar ve postoperatuar komplikasyonlar gibi özelliklerinin postoperatif ağrı şiddetini etkilemediği saptandı. Sonuç: Bupivacainle yapılan, postoperatif lokal ve bölgesel anestezinin laparoskopik kolesistektomi ameliyatlarından sonra postoperatif ağrı şiddetini azaltabileceği kanaatine varıldı.
