A Comparative Study of Trochanteric Fractures Treated With the Hip Hemiarthroplasty or the Proximal Femoral Nail

INTRODUCTION: The aim of this study was to compare the clinical and functional outcomes of patients treated with partial hip prosthesis (PHP) or double lag screw proximal femoral nailing (PFN) for unstable femoral intertrochanteric region fractures.
METHODS: In this study, the data of 101 patients who developed grade 3-4-5 femoral intertrochanteric fractures according to the Evans-jensen classification between 2019-2020 and were treated with cemented PHP or double screw PFN were evaluated retrospectively. Patients who underwent PHP were divided into Group1 and patients who underwent PFN were divided into Group 2. Data between both groups were compared statistically.
RESULTS: 101 patients evaluated within the scope of the study were divided into groups as 51 patients who underwent PHP (Group 1) and 50 patients who underwent PFN (Group 2). When the results were compared in terms of mean length of hospital stay, duration of surgery, amount of intraoperative bleeding and postoperative 1 year mortality, the results were found to be statistically significant. (p=0.00, p=0.03) While the difference in BI values between the groups at the postoperative 3rd month was statistically significant (p=0.01). Difference between the Barthel Index (BI) and Harris Hip Score (HHS) values at the 12th month was not statistically significant. (p> 0.05)
DISCUSSION AND CONCLUSION: Our study results found the PFN method superior to PHP in terms of average length of hospital stay, duration of surgery, amount of intraoperative bleeding, and postoperative 1 year mortality. Although the mean values were higher in the PFN group in terms of BI, HHS, there was no statistically significant difference.

Kalça Hemiartroplastisi ya da Proksimal Femoral Çivileme ile Tedavi Edilen Trokanterik Kırıkların Karşılaştırılmalı Bir Çalışması

GİRİŞ ve AMAÇ: Bu çalışmanın amacı instabil femur intertrokanterik bölge kırıklarında parsiyel kalça protezi (PKP) veya çift lag vidali proksimal femoral çivileme (PFN) yöntemleri ile tedavi edilmiş hastaların klinik ve fonksiyonel sonuçlarını karşılaştırmaktır.
YÖNTEM ve GEREÇLER: Bu çalışmada 2019-2020 yılları arasında Evans-jensen sınıflamasına göre evre 3-4-5 femur intertrokanterik kırık gelişen ve tedavide sementli PKP ya da çiftlag vidali PFN uygulanan 101 hastanın verileri retrospektif olarak değerlendirildi. PKP uygulanan hastalar grup 1 ve PFN uygulanan hastalar grup 2 olarak ayrıldı. Her İki grup arasındaki veriler istatistiksel olarak karşılaştırıldı.
BULGULAR: Çalışma kapsamında değerlendirilen 101 hasta PKP yapılan (Grup 1) 51 hasta ve PFN yapılan (Grup 2) 50 hasta olarak gruplara ayrıldı. Gruplar arasında ortalama yatış süresi, ameliyat süresi, intraoperatif kanama miktarı, postoperatif 1. yıl mortalite açısından sonuçlar karşılaştırıldığında sonuçlar istatistiksel olarak anlamlı bulundu. (p=0.00, p=0.03) Postoperatif 3. Ayda gruplar arasında Bartel Indeks (BI) değerleri farkı istatistiksel olarak anlamlı iken (p=0.01) 12. Aydaki BI ve Harris Kalça Skoru (HKS) değerleri arasındaki fark istatistiksel olarak anlamlı değildi. (p> 0.05)
TARTIŞMA ve SONUÇ: Çalışma sonuçlarımız ortalama yatış süresi, ameliyat süresi, intraop kanama miktarı, postop 1. yıl mortalite açısından PFN yöntemini PKP’ye göre üstün bulmuştur. BI, HKS açısından PFN yapılan Grupta ortalama değerler daha yüksek olmasına rağmen istatistiksel olarak anlamlı fark yoktur.
