The Early Term Results Of The Patients With Tibia Plateau Fractures And Treated With Ilizarov Method

Aim:The clinical and radiological results of the patients with tibial plateau fractures, who were treated with the Ilizarov method and applied to our clinic, were evaluated in our study. Method: Six male cases were retrospectively evaluated in the study. 1 piece of C ring to the femoral distal in the fracture side of the all patients with 1 cm to the distal of the knee joint, 2 cm to the distal of the fracture and a full-ring to the tibia distal metaphysis were detected with the two pieces of Kirschner wire after the closed reduction of the fracture. The etiology of the fracture, the presence of the open fractures and the complications were noted. Hohl classification was used in radiologic evaluation. Clinical evaluation in the cases was performed according to the KOOS scoring system. Findings: The mean age of the patients was 21.5 (20-24). Falling from height in 4 (66%) cases and the traffic accident in 2 (33%) cases were etiologically found. It was evaluated that the fracture of the two of our cases was type II and the fracture of one case was type I open fracture according to the classification of Gustilo Andersen. All of the cases were radiologically type IV and V according to the classification of Hohl. Good and excellent results were clinically obtained in our cases who were followed on average 26 (16-38) months. Complete bone-healing was radiologically observed in all of the cases. For all cases, closed reduction with the ligamentotaxis has been provided using Ilizarov method without implementation of any open surgical intervention. Result: It was concluded that the Ilizarov method was a good alternative for open reduction in the treatment of the patients with tibial plateau fracture because of its successful results and its ability to provide the closed reduction.

İlizarov Yöntemi ile Tedavi Edilen Tibia Plato Kırıklı Hastaların Erken Dönem Sonuçları

Amaç: Çalışmamızda kliniğimize başvuran ve İlizarov yöntemi ile tedavi edilen tibia plato kırıklı hastaların klinik ve radyolojik sonuçları değerlendirilmiştir. Yöntem: Retrospektif olarak altı erkek vaka çalışma kapsamında değerlendirildi. Bütün hastaların kırık tarafındaki femur distaline 1 adet C halka, diz ekleminin 1 cm distaline, kırığın 2 cm distaline ve tibia distal metafizine birer adet tam halka, kırığın kapalı redüksiyonu sonrası ikişer adet Kirschner teli ile tespit edildi. Kırık etyolojisi, açık kırık varlığı ve komplikasyonlar not edildi. Radyolojik değerlendirmede Hohl sınıflaması kullanıldı. Vakalarda klinik değerlendirme KOOS skorlama sistemine göre yapıldı. Bulgular: Hastaların ortalama yaşı 21.5 (20-24) di. Etyolojik olarak 4 (%66) vakada yüksekten düşme, 2 (%33) vakada ise trafik kazası saptandı. Gustilo Andersen sınıflamasına göre vakalarımızın ikisinin kırığı tip II, birinin kırığı tip I açık kırık olarak değerlendirildi. Radyolojik olarak tüm vakalar Hohl sınıflamasına göre Tip IV ve V idi. Ortalama 26 (16-38) ay takip uygulanan vakalarımızda klinik olarak iyi ve mükemmel sonuçlar elde edildi. Vakalarımızın hepsinde radyolojik olarak tam kaynama saptandı. Tüm vakalarda İlizarov yöntemi ile kapalı olarak ligamentotaksisle redüksiyon sağlanmış olup, açık cerrahi girişim uygulanmamıştır. Sonuç: Tibia plato kırıklı hastaların tedavisinde kapalı olarak redüksiyon sağlanabilmesi ve başarılı sonuçları nedeniyle İlizarov yönteminin, açık redüksiyonun iyi bir alternatifi olduğu kanaatine varılmıştır.
