Histopathological Examinations of Adult Autopsies in Istanbul Between The Years 1998-2002

Aim: Though there is a rapid progress in medical sciences, new laboratory and monitoring techniques are entering the scene every day, but autopsy still remains the most important tool in collecting and transfering information. Methods: 5616 histopathologically examined cases of age over 18 from entire 14647 autopsies performed at the Morgue Department of the Council of Forensic Medicine between the years 1998-2002 were studied according to distribution of age, sex, cause of death, pathology of internal organs, and contribution of histopathological examination to the determination of the cause of death was evaluated. Results: Male/Fexmale ratio of our cases is 4.3/1. Histopathological assessment is significant in determining the direct cause of death in 2188 cases (38.97%); and has a supporting role in explaining the cause of death in 2414 cases (42.98%). Histopathological findings are non-spesific in diagnosis in 634 cases (11.28%), and no results can be obtained with histopathological and toxicological examination in 380 cases (6.77%). Considering the cause of death, cardiovascular diseases take the first place with 1277 cases (58.36%) and the second large group considering the causes of deaths consists of respiratory system diseases with 378 cases (17.27%). Conclusion: Histopathological examination has great importance in the determination of basic causes of deaths.

1998-2002 Yıları Arasında İstanbul’daki Erişkin Otopsilerinin Histopatolojik İncelenmesi

Amaç: Tıp bilimindeki hızlı gelişime, her gün yeni katılan laboratuar ve görüntüleme tekniklerine karşın otopsi; bilgi toplama ve aktarmadaki önemini halen korumaktadır. Yöntem: Bu çalışmada İstanbul’da Adli Tıp Kurumu Morg İhtisas Dairesinde 1998-2002 yıllarında otopsisi yapılan 14647 adet olgudan, histopatolojik inceleme yapılan 18 yaş ve üzeri 5616 olgı, yaş, cinsiyet, ölüm nedenleri, iç organ patolojileri ve histopatolojik değerlendirmenin ölüm nedeninin belirlenmesine katkısı açısından değerlendirilmiştir. Bulgular:Olgularımızda Erkek/Kadın oranı 4,3/1‘dir. Olgular histopatolojik değerlendirmenin ölüm nedeninin belirlenmesine olan katkısı açısından incelendiğinde 2188 olguda (%38,97) histopatolojik değerlendirme direkt ölüm nedenini ortaya koymada belirleyici rol oynamakta, 2414 olguda (%42,98) ise ölüm nedenini açıklamada destekleyici rol oynamaktadır. Altıyüzotuzdört (%11,28) olguda histopatolojik bulgular tanı açısından belirleyici bir önem taşımamaktadır. Üçyüzseksen (%6,77) olguda ise histopatolojik, toksikolojik incelemeye karşın sonuç elde edilmemiştir. Olgularda ölüm nedenleri değerlendirildiğinde birinci sırayı 1277 olgu ile (%58,36) kardiovasküler hastalıklar almaktadır. Ölüm nedenleri açısından baktığımızda ikinci büyük grubu solunum sistemi hastalıkları oluşturmaktadır. Bu grupta olgu sayısı 378 (%17,27)’dir. Bunların dışında infeksiyon hastalıkları ve serebrovasküler hastalıklar ölüm nedenleri açısından diğer önemli gruplardır. Sonuç: Histopatolojik inceleme, temel ölüm nedenlerinin belirlenmesinde büyük öneme sahiptir.
