Amifostine Administration in Patients with Head and Neck Carcinomas

Objective: The purpose of this study was to evaluate the effects of Amifostine on acute radiotherapy effects and tolerability. Materials and Methods: 48 patients with head and neck carcinoma were included in the study. Amifostine was applied to 24 patients out of 48 patients. 300 mg/m2 Amifostin in 150 cc NaCl was given during 15 minutes via intra venous injection for 15 minutes before each conventional fraction of radiotherapy. Median total tumor was 68 (60-70) Gy. Median follow up is 7 months. Radiotherapy-related acute morbidity was scored according to EORTC/RTOG acute morbidity scoring criteria. Results: Grade 1-2 and Gr 3 mucositis, 50%, 12.5%’ e 58.3%, 16.6%, Gr1-2 dermatitis 45.8%, 20.8%’e 54.1%, 20.8%. Gr 1-2 and Gr3 disphagia 45.8% and 12.5% have been found in both groups. Grade 1-2 and grade 3 xerostomia 66.6%, 20.8% ‘e 75%, 25% were observed. In terms of statistic there weren’t any significant differences determined in both groups. Conclusion: 300 mg/m2 Amifostine has been found tolerable.

Baş Boyun Karsinomlu Hastalarda Amifostin Uygulanması

Amaç: Bu çalışmada Amifostinin akut radyoterapi toksisitesi üzerine olan etkisi ve tolerabilitesinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: 48 baş-boyun karsinomlu hasta çalışmaya alındı. 24 hastaya Amifostin verilirken 24 hastaya uygulanmadı. Konvansiyonel fraksiyonel radyoterapiden 15 dakika önce 150 cc izotonik NaCl içine 300 mg/m2, ye Amifostin 15 dakika süresince intra venöz uygulandı. Median total tümör dozu 68 (60-70) Gy idi. Akut toksisite EORTC/RTOG kriterlerine göre değerlendirildi. Takip süresi 7 aydır. Bulgular: Amifostine uygulanan ve uygulanmayan gruplarda Grade 1-2 ve Gr 3 mukozit oranları sırasıyla %50, %12.3’e %58.3, %16.6,Gr 1-2 ve Gr 3 dermatit%45.8, %20.8’e %54.1. %20.8,Gr1-2 veGr3 disfaji oranları ise her iki grupta %45.8 ve %12.5 idi. Gr 1-2 ve Gr 3 kserostomi ise %66.6, %20.8’e %75,%25 bulundu. Her iki grupta istatiksel anlamlı fark gözlenmedi. Sonuç: 300 mg/m2 Amifostin uygulanması tolerabl bulunmuştur.
