Cerebellar Rehabilitation

Receipt of a referral of a client with cerebellar dysfunction can be a frustrating event for a specialist. The first problem facing the specialist is the proper recognition and evaluation of motor problems peculiar to cerebellar disease. Second, a treatment program with known beneficial effects for the improvement of the motor deficits must be implemented. In solving these two problems a therapist can run into difficulty for a number of reasons. Although specialists typically learn a list of signs and symptoms accompaning cerebellar disease, they are not taught precise methods of examination and quantification of these symptoms. In addition, the relative significance and progression of each symptom have not been clearly defined. Last is the limited knowledge of the mechanisms underlying each motor symptom in cerebellar disease. This lack of awareness is the most serious limitation because it hampers development of effective treatment programs. This review describes, cerebellar dysfunction with regard to evaluation and treatment procedures.

Serebellar disfonksiyonlu bir kişinin tedavi reçetesi, bir uzman için boşa giden bir durum olabilir. Karşılaşılan ilk problem, serebellar hastalığa özgü, motor problemlerin uygun tanımlaması ve değerlendirmesidir. İkincisi, motor defisitlerin düzelmesi için faydalı etkileri olan bir tedavi programının yürütülmesidir. Bu iki problemi çözmede, sayısız nedenlerden dolayı zorlukla karşılaşılabilir. Serebellar hastalığa eşlik eden bulgu ve semptomlar bilinmesine rağmen, bu semptomların muayene ve niteliğinin kesin metodları bilinmez. Ayrıca, her semptomun nisbi önemi ve progresyonu açık olarak belirlenmemiştir. Sonuncusu, serebellar hastalıkta her motor semptomun altında yatan mekanizmaların bilinmesi sınırlıdır. Bu bilginin eksikliği çok önemli bir sınırlamadır çünkü etkili tedavi programının gelişmesine engel teşkil eder. Bu derleme, serebellar disfonksiyonu değerlendirme ve tedavi yaklaşımlarına göre açıklamaktadır.
